Who is Gravie?
Gravie Administrative Services LLC is a licensed Third Party Administrator that administers self-funded medical plans for employers. Benefits are administered through Gravie with leased networks through PreferredOne, Cigna and PHCS/Multiplan.
How Do I Get in Touch?
For benefits, eligibility and claims status call Gravie Provider Services:
- Mon-Fri: 8AM - 12PM CT and 1PM - 5PM CT
For all pharmacy related questions call the pharmacy administrator Magellan RX:
- Sun-Sat, 24 hrs a day
Where Do I Submit Claims?
Look for the network logo on the insurance card and follow the instructions below:
- PreferredOne providers submit claims electronically using the EDI payor ID GRAVIE or mail claims to P.O. Box 1527, Minneapolis, MN 55440-1527.
- Non-PreferredOne providers submit claims by mail to Gravie Administrative Services LLC, PO BOX 3465, Minneapolis, MN 55403.
- All providers outside Minnesota follow the instructions below for PHCS/Multiplan.
All providers submit claims electronically using the EDI payor ID 62308 or mail claims to P.O. Box 188061, Chattanooga, TN 37422-8061.
All providers submit claims electronically using the EDI payor ID GRAVIE or mail claims to Gravie Administrative Services LLC, PO BOX 3465, Minneapolis, MN 55403.

WHAT TO DO IF A PRIOR AUTHORIZATION IS REQUIRED
Claims requiring a Prior Authorization will reject with an error code of “75 — Prior Authorization Required” alerting you that additional requirements are needed to fill the requested prescription. You will need to inform the member that additional information is required from the member’s prescribing doctor.
You may provide the Prescription Drug Prior Authorization Request form to the member and direct the member to contact their prescribing doctor to complete the form and fax it to Magellan RX Management at 800-424-3260. The doctor may need to contact Magellan RX Management to discuss whether the intended use of the drug is allowable on the member’s plan.
Please alert the member that the above steps will take additional time to get the prescription filled. If this is an urgent prescription, have the member call Magellan Customer Service at 800-424-3312.
Pharmacy will only accept original prescription orders from patients.
Faxed prescriptions can be accepted from the prescribing physician if faxed to the member’s pharmacy.
